The blood cerebrospinal fluid barrier orchestrates immunosurveillance, immunoprotection, and immunopathology in the central nervous system

Recent research made possible by technological advances has redefined the central nervous system (CNS) as a somewhat immunologically active and specialized system. Complex CNS barriers modulate the immunosurveillance and immune cell trafficking which is fundamental for normal physiological and behavioral activities as well as CNS immune homeostasis. The anatomical positioning of the blood-brain barrier (BBB), blood leptomeningeal barrier (BLMB), and blood cerebrospinal fluid barrier (BCSFB) firmly modulates CNS homeostasis in both health and disease states. The BCSFB, a major immune checkpoint comprised of the monolayered choroid plexus (ChP) and cerebrospinal fluid (CSF), is the one that controls the communication between the CNS and periphery. Based on emerging evidence, here, we evaluated both the anatomical localization and physiological functions of the CNS barriers, focusing on the BCSFB function in CNS homeostasis and communication. Moreover, the ChP and CSF roles as the key elements for neuroimmune communication are underscored. Lastly, the suitability of the BCSFB as a therapeutic target in various neurological anomalies such as multiple sclerosis, major depressive disorder, and Alzheimer’s disease is discussed.
